Alachua County encourages residents to respond to vaccine survey

In partnership with the WellFlorida Council (the local health planning council), the Alachua County Board of County Commissioners invites you to answer this COVID-19 Vaccine Survey. We are seeking 10,000 surveys. The survey will be available from March 12 to March 26 at 5 p.m. The survey may close earlier than March 26 if 10,000 responses have been recorded. 

COVID-19 vaccine distribution is an evolving and rapidly changing process and may, in fact, change during the time this survey is available. Despite this challenge, Alachua County leaders feel it is vital to hear from residents to better understand fundamental factors, challenges, and perspectives on the vaccine and vaccine delivery in our county. Community leaders will use your input to help guide decisions about COVID-19 vaccine distribution in Alachua County. 

This survey has approximately 30 questions and should take about 10-15 minutes to finish. Your answers will not be used to identify you. Survey information:

  • You must be at least 18 years old to take the survey
  • You must be an Alachua County resident to take the survey
  • Please answer all questions within the survey
  • The survey will automatically close once 10,000 responses have been recorded
  • Please answer the survey only once
  • The survey is only accessible online (phone, computer, tablet)
